The cost pathway starts with eligibility and mapping
SIRT is usually not priced as a single isolated procedure. The center may need specialist review, current imaging and blood tests before scheduling a mapping angiography with a test tracer and nuclear-medicine imaging. Those steps are clinically necessary because the treatment plan can change before therapeutic microspheres are ordered or administered.
Major cost drivers
Ask the hospital to specify which parts of the pathway are included in the estimate.
- Interventional radiology and nuclear-medicine review
- Pre-treatment CT/MRI and laboratory testing if required
- Mapping angiography
- Coil embolization of extrahepatic branches when needed
- 99mTc-MAA test administration and SPECT/CT
- Dosimetry and treatment planning
- Y-90 microspheres and prescribed activity
- One-lobe versus staged bilobar treatment
- Inpatient nuclear-medicine stay
- Post-treatment imaging and laboratory monitoring
- Management of catheter or radiation-related complications
Ask what is billed if mapping shows that SIRT is unsuitable
The pre-treatment evaluation can identify vascular anatomy, shunting or other findings that make the planned radioembolization inappropriate. International patients should ask how the center bills the completed diagnostic mapping if therapeutic treatment is cancelled or changed.
A staged treatment can create a second hospital episode
Charité notes that some patients are treated in separate sessions for the two liver lobes. A quotation should make clear whether it assumes one treatment, staged treatment, or leaves a second session to be priced after the first treatment and response assessment.
Budget for travel and follow-up outside the hospital invoice
International patients may need accommodation around separate evaluation and treatment dates, repeat laboratory tests, a companion, and follow-up CT or MRI after returning home. These costs may be outside the German hospital estimate even though they are part of the practical treatment pathway.
