Myasthenia Gravis Treatment in Germany: Diagnosis, Immunotherapy & Thymectomy

Understand how German neuromuscular centers diagnose and treat myasthenia gravis, including antibody testing, neurophysiology, pyridostigmine, immunotherapy, thymus assessment, thymectomy and crisis care.

Short answer: Myasthenia gravis is an autoimmune disorder of neuromuscular transmission that can cause fluctuating weakness of the eyes, face, swallowing, speech, breathing muscles or limbs. German specialist centers combine antibody testing and neurophysiology with clinical assessment, review for thymic disease where appropriate and individualize treatment with symptomatic medication, immunotherapy and selected thymectomy. A myasthenic crisis with respiratory or severe swallowing weakness is an emergency and should be treated immediately where the patient is located rather than managed as planned international travel.

Confirm the diagnosis before escalating treatment

Fluctuating weakness can have several causes. Charité and Freiburg describe structured diagnostic pathways using the clinical pattern together with disease-specific antibody testing and electrophysiology. A second opinion is particularly useful when symptoms and previous tests do not fit together or when the diagnosis remains uncertain.

  • AChR antibody testing
  • MuSK antibody testing when clinically appropriate
  • LRP4 and other antibody testing in selected seronegative cases
  • Repetitive nerve stimulation
  • Single-fiber EMG or other specialized neurophysiology when needed
  • Review for alternative neuromuscular disorders such as LEMS or congenital myasthenic syndromes

Ocular and generalized disease are different treatment situations

Some patients initially have ptosis or double vision only, while others develop bulbar, respiratory or limb weakness. The distribution, severity, antibody status, age, comorbidity and previous treatment response influence the treatment plan and the urgency of escalation.

Pyridostigmine is a symptomatic treatment, not the whole pathway

Freiburg describes pyridostigmine as a symptomatic option used after specialist assessment. Many patients with clinically significant generalized autoimmune myasthenia also require immunotherapy because improving neuromuscular transmission alone does not address the autoimmune process.

Immunotherapy is individualized

German myasthenia centers evaluate immunosuppressive treatment according to disease severity, antibody subtype, previous response, adverse effects and other medical conditions. The exact regimen can include established immunosuppressive approaches and, for selected patients, newer targeted therapies. A specialist review should document what has already been tried and why treatment was changed or stopped.

Thymus assessment matters in selected patients

Myasthenia can be associated with thymic abnormalities including thymoma. Chest imaging and thoracic-surgery review may therefore become part of the pathway. Freiburg specifically describes cooperation with thoracic surgery for thymectomy, including minimally invasive approaches. Thymectomy is not an automatic recommendation for every patient and should be linked to thymoma status, disease subtype and the specialist treatment plan.

Crisis treatment is fundamentally different from elective care

Freiburg describes inpatient and, when necessary, intensive-care escalation with intravenous immunoglobulins, plasmapheresis or immunoadsorption for threatened or established crisis. Charité and Heidelberg also emphasize that severe myasthenia can become dangerous. New breathing difficulty, rapidly worsening swallowing, inability to handle secretions or marked generalized weakness requires urgent local assessment.

Pregnancy and other special situations need coordinated planning

Freiburg provides counseling before and during pregnancy for patients with myasthenia. Medication choices, infection risk, surgery timing and obstetric planning can require coordination between neurology and other specialties. International travel should be planned around the clinical situation rather than around a generic treatment package.

What to send for a German myasthenia second opinion

A concise record package should allow the center to verify the diagnosis and understand treatment response over time.

  • Neurology and neuromuscular-clinic reports
  • AChR, MuSK, LRP4 and other antibody results
  • Repetitive nerve stimulation, single-fiber EMG or other neurophysiology reports
  • Chest CT/MRI and thymus findings when performed
  • Current pyridostigmine and immunotherapy regimen
  • Previous steroids, immunosuppressants, IVIG, plasma exchange or targeted-therapy history
  • History of prior crisis, ICU admission or respiratory support
  • Thymectomy or thoracic-surgery records if previously treated
  • A specific question: confirm diagnosis, optimize therapy, assess thymectomy or review refractory disease

Charité Berlin – Integrated Myasthenia Center

Berlin · Charité publishes a certified interdisciplinary myasthenia center with antibody testing, electrophysiological diagnostics, differential diagnosis of related neuromuscular syndromes, second opinions and comprehensive treatment of myasthenia gravis.

University Medical Center Freiburg – Myasthenia / Neuroimmunology Clinic

Freiburg · Freiburg documents a specialist myasthenia pathway covering AChR, MuSK, LRP4 and Titin antibody testing, repetitive nerve stimulation, pyridostigmine, immunosuppression, thymectomy coordination and escalation with IVIG, plasmapheresis or immunoadsorption when clinically necessary.

Heidelberg University Hospital – Myasthenia Outpatient Clinic

Heidelberg · Heidelberg publishes a dedicated myasthenia gravis outpatient pathway and notes that the disease can affect ocular, bulbar and limb muscles and can progress to crisis requiring temporary intensive care.
