Diagnostic clarification can be the first separate cost episode
When Alzheimer’s disease has not been securely established, a German memory clinic may need to review the cognitive history, perform neuropsychological testing and reassess brain imaging and laboratory data before discussing treatment.
- Memory-clinic or neurocognitive specialist consultation
- Neuropsychological testing
- Brain MRI or CT review
- Laboratory testing for reversible or contributory causes
- Follow-up consultation to explain the diagnosis and treatment plan
Biomarker confirmation creates another diagnostic layer
Selected patients may need CSF Alzheimer biomarkers and/or amyloid imaging. This is particularly important when disease-modifying anti-amyloid treatment is being considered because lecanemab requires confirmed amyloid pathology under its authorised indication.
Lecanemab is a repeated-treatment pathway, not a one-time infusion
Freiburg describes outpatient lecanemab administration every two weeks with regular follow-up and imaging. The total budget therefore depends on the number of infusions, infusion-unit fees, specialist review, laboratory work and the MRI safety schedule. Ask the center to separate eligibility work-up from the treatment course itself.
- Eligibility and ApoE4 assessment
- Baseline MRI and amyloid confirmation
- Repeated infusion visits
- Required MRI safety scans
- Neurological or memory-clinic follow-up
- Management of infusion reactions or suspected ARIA if they occur
Standard long-term care continues outside the infusion budget
Medication review, cognitive and physical activity, psychiatric care, social support, caregiver counseling and management of vascular risk can continue over years. These ongoing costs should not be confused with a short German diagnostic visit or an anti-amyloid treatment estimate.
Travel logistics can be unusually important for anti-amyloid therapy
A therapy requiring repeated infusions and MRI monitoring may be difficult to manage entirely through international travel. Before budgeting flights or accommodation, confirm whether the German center can provide the full course, whether treatment capacity is available and whether a safe shared-care arrangement with clinicians at home is possible.
Ask for an estimate only after the clinical pathway is defined
The most useful quotation is based on the actual question: diagnostic second opinion, biomarker confirmation, standard dementia care or selected disease-modifying therapy.
- Send prior cognitive testing and specialist reports
- Send MRI/CT images and reports
- Send CSF or amyloid-PET results if available
- Send ApoE genotype if already tested
- List anticoagulants, antiplatelet medicines and major neurological comorbidities
- Ask whether the estimate covers all required MRI and infusion visits
