The transplant technique is the main cost driver
Corneal transplantation is not one standardized operation. Posterior lamellar transplantation such as DMEK or DSAEK, deep anterior lamellar keratoplasty (DALK) and full-thickness penetrating keratoplasty use different donor tissue, surgical steps and follow-up pathways. A price comparison is meaningful only after the German cornea team has identified which operation is actually planned.
- DMEK for selected endothelial disease
- DSAEK for selected posterior corneal disease
- DALK when diseased anterior stroma can be replaced while preserving healthy endothelium
- Penetrating keratoplasty when full-thickness replacement is required
Preoperative corneal diagnostics should be part of the estimate
A transplant plan should be based on the actual corneal anatomy and previous eye surgery. Ask whether the quotation includes the specialist review and measurements needed to confirm the operative strategy.
- Cornea-specialist consultation and slit-lamp examination
- Corneal topography or tomography when indicated
- Pachymetry / corneal-thickness measurement
- Endothelial assessment or specular microscopy when relevant
- Refraction and visual-acuity testing
- Review of earlier cataract, glaucoma, refractive or corneal surgery
Donor tissue and cornea-bank logistics belong in the financial plan
German university centers may coordinate transplantation through a cornea bank, including donor-tissue preparation and quality assurance. International patients should ask whether donor tissue is included in the estimate, whether tissue availability can affect the operation date and what happens financially if the planned date changes.
DMEK follow-up may include additional early procedures
DMEK commonly uses an air or gas bubble to attach the graft. If the graft partially detaches, an additional air injection (rebubbling) may be required. Ask whether the initial estimate includes only the transplant itself or also covers early checks, pressure monitoring and any planned policy for rebubbling.
Penetrating keratoplasty has a different follow-up horizon
Full-thickness keratoplasty can involve corneal sutures that remain for many months. The operation-day price therefore does not describe the whole care pathway. International patients should clarify which early reviews must occur in Germany, how suture management will be organized and which long-term follow-up can safely be transferred to an ophthalmologist at home.
Combined eye procedures can change the quotation substantially
Some eyes need additional treatment such as cataract surgery, intraocular-lens work, glaucoma management or treatment of ocular-surface disease. These services should be listed separately instead of being assumed to be included in a generic corneal-transplant package.
Ask what the written estimate includes and excludes
A useful comparison is between quotations with the same clinical scope, not between headline numbers.
- Planned keratoplasty technique
- Donor tissue and tissue-preparation costs
- Surgeon, operating-room and anesthesia costs
- Outpatient versus inpatient care and assumed length of stay
- Postoperative eye drops and prescriptions
- Number and timing of early postoperative examinations
- Policy for rebubbling, additional surgery or unexpected findings
- Longer-term suture and graft follow-up
Why we do not publish a made-up fixed euro range
LMU Klinikum explains that it does not publish a general treatment price list and that the final cost depends on the complexity and services actually provided. Its international-patient process uses a case-specific cost estimate and advance payment. For corneal transplantation, a written estimate prepared after specialist review is therefore more useful than an unsupported fixed online price.
