The quotation should follow a corneal and lens assessment
Fuchs dystrophy can coexist with cataract, glaucoma, macular disease or previous eye surgery. Before pricing DMEK, the center should confirm how much of the visual loss is caused by endothelial failure and whether lens surgery should be performed separately or together with the transplant.
- Cornea-specialist consultation
- Slit-lamp examination and visual acuity
- Pachymetry / corneal-thickness measurement
- Endothelial cell assessment or specular microscopy when useful
- Corneal topography or tomography when indicated
- Lens and cataract assessment
- Retinal OCT or glaucoma testing when another cause of visual loss is suspected
DMEK and combined cataract-DMEK are different cost pathways
Some patients need DMEK alone, while selected patients with clinically relevant cataract may undergo a combined cataract operation and DMEK. A written estimate should state whether the lens procedure, intraocular lens, biometry and any refractive planning are included or billed separately.
Ask what is included in the transplant episode
DMEK is more than the operating-room step. The estimate should identify donor-tissue handling, anesthesia, medicines and the early postoperative pathway rather than presenting a single unexplained procedure fee.
- Donor corneal tissue and preparation
- DMEK procedure and operating-room costs
- Local or general anesthesia when required
- Air or gas used to attach the graft
- Postoperative eye drops
- Early intraocular-pressure checks
- First-day and first-week examinations
Rebubbling can change the final cost
The thin DMEK graft can partially detach after surgery. Charité and other university cornea centers describe additional air or gas injection as a possible treatment when the graft does not remain fully attached. International patients should ask whether a rebubbling procedure is included in the estimate or charged separately and how long they should stay near the center.
DSAEK or full-thickness keratoplasty should not be priced as if they were DMEK
If DMEK is unsuitable because of anatomy, scarring or previous surgery, the treating team may recommend DSAEK or another transplant strategy. These are different procedures with different materials, recovery and follow-up requirements, so a quotation should name the exact operation rather than use a generic 'corneal transplant' price.
Travel and follow-up can be a meaningful part of the real budget
International patients may need to remain near the center for early checks because graft attachment, eye pressure and inflammation are monitored closely after DMEK. Accommodation, transport, medication and later local ophthalmology follow-up may therefore matter as much as the headline surgical fee.
Why we do not publish an invented fixed euro range
LMU Klinikum explains that it does not publish a general treatment price list and that final treatment costs depend on the complexity and services actually provided. The safest comparison is therefore between written hospital estimates that describe the same transplant, cataract strategy and follow-up scope.
