The diagnosis determines the cost pathway
A patient with focal cervical dystonia treated mainly with repeated injections has a different care plan from a patient with severe generalized dystonia being assessed for DBS. The hospital therefore needs the clinical pattern and previous treatment history before a quotation can be interpreted.
Botulinum-toxin treatment is usually a recurring outpatient cost
For many focal dystonias, treatment involves specialist assessment, muscle selection and repeated injections because the effect is temporary. International patients should clarify the expected review interval and whether future injections can be continued locally after the initial German consultation.
- Movement-disorder consultation
- Botulinum-toxin product and dose
- Injection procedure and guidance technique when used
- Follow-up to assess benefit and side effects
- Expected frequency of repeat treatment
Diagnostic testing is individualized
Some patients need only a specialist clinical review, while others may require MRI, laboratory testing, genetics or other investigations because the dystonia pattern suggests a secondary or inherited cause. These tests should not be assumed to be part of a standard package unless they are listed in the written estimate.
DBS is a substantially different cost episode
A DBS quotation can include pre-operative neurological and neurosurgical assessment, imaging, implanted electrodes and pulse generator, anesthesia, surgery, inpatient care and initial programming. Device choice and hospital course can affect the final estimate.
- Neurology and functional-neurosurgery candidacy review
- Pre-operative brain imaging and other planned assessments
- DBS electrodes, extensions and implanted pulse generator
- Operating-room and anesthesia services
- Expected inpatient stay
- Initial postoperative device programming
- Policy for complications or a longer admission
Long-term programming and hardware management continue after surgery
DBS should not be budgeted as a one-time operation. Stimulation settings may require repeated adjustment, and implanted hardware can eventually require maintenance or generator replacement depending on the system. Before traveling, ask which follow-up visits are included and how later programming can be coordinated closer to home.
Rehabilitation and supportive treatment may be billed separately
Physiotherapy, occupational therapy, speech or swallowing therapy and pain treatment may be useful for selected dystonia patterns but are not automatically included in a neurological consultation or surgical estimate. Clarify which services belong to the hospital episode and which are separate.
Send detailed previous-treatment records before requesting a quotation
A center can estimate the likely pathway more accurately when it can see what has already been tried.
- Movement-disorder reports and videos
- Medication history
- Botulinum-toxin injection records including muscles and doses when available
- MRI and other prior investigation results
- Genetic testing if previously performed
- Previous DBS or surgery reports and device details
- The specific purpose of the inquiry: second opinion, injection optimization or DBS assessment
