Why myeloma cannot be priced as one procedure
Heidelberg Myeloma Center publishes a broad treatment spectrum including systemic drug combinations, high-dose therapy with autologous stem-cell transplantation, CAR-T, allogeneic transplantation, radiotherapy, supportive care and clinical trials. These pathways differ substantially in duration, resource use and monitoring, so one online 'myeloma price' would combine clinically different treatment episodes.
The estimate starts with the current disease state
A realistic quotation requires the hematology team to know whether the disease is newly diagnosed, relapsed or refractory, which prior therapy lines were used, whether transplant or CAR-T has already been performed, and whether kidney dysfunction, infection, bone disease or other complications require additional care.

Drug combinations can include several medicines, repeated treatment cycles and laboratory monitoring. The total depends on the exact regimen, whether high-cost antibodies or other novel medicines are used, the number of cycles and whether complications or transfusion support require inpatient care.
Autologous stem-cell transplant has its own cost structure
For a patient being considered for high-dose therapy and autologous transplant, the financial episode can include mobilization and collection of stem cells, conditioning chemotherapy, inpatient admission, transfusion and infection support, monitoring until blood-count recovery and early follow-up. It should not be compared directly with an outpatient drug-treatment quotation.
CAR-T should be quoted as a complete cellular-therapy pathway
Ulm and Freiburg publish CAR-T treatment for selected multiple-myeloma patients. A useful estimate should consider eligibility assessment, cell collection, product manufacturing, possible bridging treatment, lymphodepleting therapy, infusion, inpatient monitoring, management of serious toxicity and required follow-up rather than presenting the cell product as the only cost item.
Hospital estimates can change if the clinical course changes
Ulm University Hospital states that international self-pay cases are calculated individually from comprehensive medical findings, inpatient estimates are based on the German DRG system, advance payment is required before treatment scheduling and additional treatment can lead to an additional advance invoice. The final invoice can therefore differ from the original advance invoice.
Questions to ask before paying
Compare the assumed clinical episode, not only the total figure.
- Which myeloma treatment regimen does the estimate assume?
- Are high-cost medicines and supportive medicines included?
- Does the estimate include bone-marrow review and required imaging?
- If transplant is planned, are collection, conditioning and the full inpatient stay included?
- If CAR-T is being evaluated, is the quote for assessment only or the complete treatment pathway?
- What inpatient duration is assumed?
- How are ICU care, infections, transfusions or other complications billed?
- Which follow-up visits and tests are included?
- How are unused advance payments or additional charges settled after treatment?
Plan non-hospital costs around repeated specialist care
Myeloma care can require repeated trips, a prolonged stay after transplant or cellular therapy, and frequent laboratory monitoring. Ask how long the patient and companion may need accommodation near the center and which follow-up can safely be transferred to the home-country hematology team.
