Diagnostic re-classification can be a major first cost episode
Patients referred with a broad or historical diagnosis such as 'polymyositis' may need targeted re-evaluation before treatment is changed. The estimate may include specialist consultations, laboratory testing, electrophysiology, imaging and pathology review.
- Neuromuscular or neuroimmunology consultation
- CK and other laboratory tests
- Myositis-specific and associated antibody testing
- EMG/ENG
- Muscle MRI or ultrasound
- Pathology review or new muscle biopsy when indicated
Organ screening changes the work-up budget
Dermatomyositis and antisynthetase-spectrum disease can require evaluation beyond muscle. Depending on the subtype and symptoms, additional dermatology, rheumatology, pulmonary testing, chest imaging, swallowing studies or individualized malignancy screening can materially change the estimate.
Immunotherapy cost depends on the actual treatment plan
Immune-mediated myopathies may require corticosteroid, immunomodulatory, immunosuppressive or selected biologic strategies, but there is no universal regimen. Drug choice, dose, body weight, infusion schedule, inpatient versus outpatient administration, laboratory monitoring and infection-risk management can all affect cost. Ask for an episode-specific estimate after the specialist has confirmed the subtype.
IVIG and infusion-based treatment should be quoted separately
When intravenous immunoglobulin or another infusion therapy is considered, the relevant budget can include medication quantity, day-unit or inpatient charges, pre-treatment laboratory tests and repeat sessions. Do not assume that an infusion course is included in the consultation fee.
IBM often shifts cost toward function, swallowing and long-term support
In inclusion-body myositis, repeated broad immunosuppression may not be the central treatment expense. Rehabilitation, assistive devices, fall prevention, swallowing assessment and nutrition planning may represent a larger share of useful care. The quotation should match the patient's real functional goals.
Rehabilitation is usually a separate care episode
Physiotherapy, occupational therapy, speech/swallowing therapy and inpatient or outpatient rehabilitation can be priced separately from diagnostic neurology care. Ask whether the German center expects rehabilitation to be performed locally after the specialist review or during the same visit.
Send complete records before requesting a quotation
The more complete the existing work-up, the easier it is for a center to avoid unnecessary repetition and produce a realistic estimate.
- Neurology/rheumatology reports and symptom timeline
- CK and laboratory trends
- Myositis antibody results
- EMG/ENG reports
- Muscle MRI images and reports
- Biopsy report and pathology material if available
- Pulmonary or swallowing assessments
- Current weight if weight-based infusion therapy is being discussed
- Complete previous immunotherapy history
- The purpose of the inquiry: diagnosis confirmation, treatment escalation, biopsy review or rehabilitation planning
