Diagnostic clarification may be the main first cost episode
PSP can initially overlap with Parkinson’s disease, MSA or corticobasal syndromes. A specialist review usually begins with the clinical chronology, neurological and eye-movement examination, medication response and existing MRI before deciding whether further testing is useful.
- Movement-disorder consultation
- Review of gait, falls and eye-movement findings
- Medication-response review
- MRI review
- Neuropsychology or other targeted assessment when indicated
Functional imaging should be quoted only when it adds diagnostic value
Freiburg describes selected use of dopaminergic and metabolic/receptor imaging to help classify atypical Parkinson syndromes. PET or SPECT should therefore be budgeted as an optional diagnostic layer rather than automatically included in every PSP work-up.
Medication costs are usually part of ongoing symptom management
Atypical Parkinson syndromes often respond less strongly to standard Parkinson medication than idiopathic Parkinson’s disease. The relevant budget is therefore not an advanced-procedure package but specialist medication review, monitoring and repeated adjustment according to symptoms and tolerability.
Rehabilitation can become a major recurring cost
Physiotherapy and occupational therapy may be needed repeatedly as balance, transfers and mobility change. Ask whether a German rehabilitation assessment is intended to create a home program for continuation locally or whether a longer inpatient/outpatient rehabilitation course is being proposed.
Speech and swallowing assessment should be budgeted separately
Dysarthria and dysphagia can become clinically important in PSP. Speech-language evaluation, instrumental swallowing testing when indicated, nutrition review and follow-up are distinct from the neurological consultation and may need to be coordinated locally after return home.
Send records before asking for a quotation
A center can estimate the likely pathway more accurately when it sees the chronology and existing testing first.
- Neurology reports and symptom timeline
- MRI images and reports
- Previous PET/SPECT/DaTSCAN if available
- Current and previous Parkinson medicines with response
- Fall history and current mobility aids
- Speech/swallowing or neuropsychology reports
- The purpose of the inquiry: diagnostic clarification, treatment review or rehabilitation/supportive-care planning
