The financial plan starts with the cause of implant failure
UKE separates revision arthroplasty from primary joint replacement and documents loosening, infection, wear, instability and dislocation among reasons for revision-focused evaluation. The cost estimate should therefore be based on the actual problem rather than on the generic phrase 'prosthesis exchange'.
Diagnostic work-up can be substantial before the operation
A painful prosthesis may require specialist radiograph review, additional imaging, laboratory testing and infection-focused diagnostics before the center can define the surgical plan. Ask which preoperative investigations are included in the estimate and which are billed separately.
- Orthopedic revision consultation
- Current and previous X-ray review
- CT or other imaging when clinically required
- Inflammatory markers and laboratory testing
- Joint aspiration, microbiology or other infection work-up when indicated
- Review of implant cards and previous operative reports
Implant exchange and reconstruction are not one standard operation
The planned procedure may involve exchanging one component, removing a complete prosthesis, reconstructing bone defects or using revision-specific stems, cups, augments or other implants. Charité documents complex revision hip surgery with bone-defect reconstruction and special implant systems in selected cases. The hospital quote should identify the expected revision strategy and implant requirements.
Major bone loss or repeated surgery can increase resource use
Previous operations can leave reduced bone stock and scarred soft tissues. Complex reconstruction may require longer operating time, additional implants or graft materials, blood-management planning and a more demanding postoperative course than uncomplicated primary arthroplasty.
Prosthetic-joint infection can create a different treatment sequence
When infection is confirmed or strongly suspected, treatment may require infection-directed surgery, microbiology, prolonged antimicrobial therapy and, in selected cases, a staged revision strategy. Not every infection follows the same sequence, so the estimate should specify which treatment stage is being priced rather than imply that one quotation covers every possible step.
Hospital stay may be longer than after a straightforward primary replacement
Revision arthroplasty can require closer postoperative monitoring, wound management, mobility support or treatment of complications. Ask what length of stay the estimate assumes and how additional inpatient days, intensive or intermediate care and unplanned procedures are billed.
Rehabilitation and travel home should be budgeted separately from surgery
The medical episode does not end when the implant is exchanged. International patients should clarify weight-bearing restrictions, walking aids, physiotherapy, wound review, thrombosis prevention, follow-up imaging and when long-distance travel is considered safe. Inpatient or outpatient rehabilitation may be a separate quotation from the surgical hospital stay.
The self-pay estimate remains preliminary
German university-hospital international-patient processes generally prepare an estimate after medical review and require advance payment before treatment confirmation. The final invoice reflects the care actually delivered, so unexpected implant requirements, complications, extra hospital days or additional procedures can change the total cost.
Questions to ask before advance payment
Compare hospital quotations only when they describe the same revision problem and surgical plan.
- What is the suspected cause of prosthesis failure?
- Which infection tests are included before surgery?
- Which prosthesis components are expected to be exchanged?
- Are revision-specific or custom implants included in the estimate?
- Is bone-defect reconstruction anticipated and included?
- If infection is present, which treatment stage does the quote cover?
- Are antibiotics and microbiology included where relevant?
- What inpatient duration is assumed?
- Is rehabilitation included or quoted separately?
- How are complications, extra days and final invoice reconciliation handled?
