MRI and CT can be separate planning costs
Contrast MRI defines the tumor, optic apparatus and vessel relationships, while thin-slice CT can show hyperostosis, calcification and optic-canal bone anatomy. Repeat imaging may be requested if outside studies are old, incomplete or unsuitable for skull-base planning.
Formal visual testing is part of the real treatment budget
Visual acuity, perimetry and ophthalmology or neuro-ophthalmology assessment can influence urgency and surgical planning. These services may be billed separately from the neurosurgical consultation.
Observation still requires serial imaging
A selected watch-and-scan strategy avoids immediate procedural costs but requires repeat MRI and specialist review. International patients should clarify whether future scans can be performed locally and reviewed remotely.
Transcranial surgical estimates depend on tumor anatomy
Operative time and complexity vary with tumor size, lateral extension, vessel encasement, optic-canal invasion, calcification and prior treatment. Neuronavigation, neuromonitoring, intraoperative imaging and inpatient monitoring may be incorporated into the hospital estimate.
Endoscopic endonasal surgery has a different resource profile
An endoscopic transnasal approach can involve neurosurgery and ENT, dedicated endoscopic equipment and multilayer skull-base reconstruction. The reconstruction and postoperative monitoring are important because CSF leak is a recognized complication profile of endonasal anterior skull-base surgery.
Optic-canal decompression adds technical complexity
When tumor extends into the optic canal, decompression or drilling around the canal may be part of either transcranial or selected endonasal surgery. The estimate should reflect the planned procedure rather than a generic craniotomy or transsphenoidal package.
Pathology is an additional post-operative component
If tissue is removed, neuropathology confirms diagnosis and WHO grade. Additional immunohistochemical or molecular analysis can be indicated in selected atypical or recurrent cases.
Residual tumor can create a later radiation episode
When tumor is deliberately left around critical vessels or the optic apparatus, postoperative MRI documents the residual. Later stereotactic radiosurgery or fractionated radiotherapy may be billed as a separate episode from the surgical admission.
Radiation costs depend strongly on optic-pathway constraints
Planning requires MRI/CT fusion, target contouring, optic nerve and chiasm dose constraints, dosimetry and quality assurance. If single-session radiosurgery is not suitable because of proximity to the optic apparatus, fractionated treatment changes the number of visits and total cost.
Complications can extend admission and the final invoice
CSF leak, visual deterioration, endocrine problems, vascular complications, infection or other postoperative issues can prolong monitoring or hospitalization. International self-pay patients should ask how unplanned treatment and extended stays are billed.
