The cost depends first on which TSC manifestation needs action
Epilepsy, SEGA, kidney disease, pulmonary LAM, cardiac disease, skin manifestations and TAND can generate very different diagnostic and treatment pathways. Ask the hospital to quote the current priority problem instead of requesting a generic all-organ TSC package.
Genetic confirmation may or may not need repeating
If a complete TSC1/TSC2 report already documents a pathogenic variant, repeating testing may add little. Unresolved or mosaic cases can require deeper sequencing, deletion/duplication analysis or specialist variant interpretation. Sending the full original laboratory report can prevent unnecessary duplication.
Epilepsy evaluation can range from record review to presurgical work-up
A medication-focused second opinion is much less resource-intensive than inpatient video-EEG, high-resolution MRI, neuropsychology, PET/SPECT or invasive monitoring for drug-resistant epilepsy. The estimate should specify whether the goal is medication optimization, seizure classification or epilepsy-surgery candidacy.
Brain MRI and SEGA follow-up are longitudinal rather than one-time costs
Serial imaging may be required to document growth of SEGA or other structural changes. If intervention is needed, the pathway can include neurosurgical consultation, surgery and inpatient care or systemic mTOR-inhibitor assessment and follow-up imaging.
Everolimus cost includes monitoring, not only medication
For approved TSC indications, long-term everolimus treatment can require baseline review, laboratory testing, interaction checks, adverse-effect management, therapeutic-drug monitoring in selected indications and repeated imaging or seizure assessment. The medicine price alone is therefore not the complete treatment cost.
Renal angiomyolipoma creates a separate nephrology/urology pathway
Kidney imaging, renal-function testing, aneurysm assessment and management of hemorrhage risk can involve nephrology, urology and interventional radiology. Embolization, surgery and systemic mTOR treatment have very different cost structures.
Multisystem surveillance should be targeted rather than duplicated
TSC follow-up can involve neurology, nephrology, pulmonology, cardiology, dermatology, ophthalmology and neuropsychology. A coordinated TSC center can help decide which assessments are currently due and avoid repeating recent studies without a clinical reason.
TAND and developmental assessment can be clinically important
Neuropsychological, psychiatric or developmental evaluations are separate services and may be justified by school, behavior, attention, mood, autism-spectrum, sleep or adaptive-function concerns. They should be included in a quote only when relevant to the patient's goals.
International-patient estimates should follow document review
For self-pay care, a center may first review records before deciding whether the patient needs a remote opinion, outpatient multidisciplinary visit, inpatient epilepsy work-up, surgery or medication initiation. This sequence produces a more realistic estimate than a headline price.
Send these records before requesting a TSC estimate
The more complete the records, the less likely the first visit is to repeat avoidable diagnostics.
