The current tumor burden determines the first estimate
A VHL center should identify which lesions currently need action and which can remain under surveillance. A generic package that repeats every organ test regardless of recent high-quality records is less useful than a targeted multidisciplinary plan.
Longitudinal MRI review can reduce unnecessary repeat imaging
CNS hemangioblastoma decisions depend heavily on growth and cyst formation, so prior brain and spine MRI DICOM files are valuable. If recent imaging is technically adequate, the German team may be able to perform an expert comparison before deciding whether new MRI is needed.
Hemangioblastoma surgery is a separate major cost episode
When a CNS lesion needs treatment, cost depends on brain versus spinal location, complexity, neuromonitoring, vascular imaging, selected preoperative embolization, inpatient stay and postoperative imaging. Multiple VHL lesions do not imply that all should be operated during one admission.
Belzutifan cost includes monitoring as well as medication
For adults who meet the EU indication, systemic belzutifan can require baseline laboratory and oxygenation assessment, repeated blood counts, adverse-effect monitoring, oncology/VHL follow-up and serial imaging of target lesions. Drug cost alone does not represent the full care pathway.
Kidney-preserving care has its own cost structure
VHL-associated renal tumors can require serial MRI, nephron-sparing surgery, ablation or systemic-treatment discussion. Recurrent or bilateral disease makes long-term renal-function preservation especially important, so a kidney-focused estimate should be separated from neurosurgical costs.
Pancreatic, adrenal, retinal and ear manifestations create additional specialist pathways
Pancreatic neuroendocrine tumors, pheochromocytoma, retinal hemangioblastoma and endolymphatic sac tumors require different imaging, laboratory tests and procedures. They should be included in the quote only when currently relevant.
Genetic counseling and family testing may be separate services
If the familial pathogenic VHL variant is already known, targeted testing of relatives may be simpler than full diagnostic sequencing. New index cases or unresolved variants may require broader genetic assessment and counseling.
Coordinated VHL review can reduce fragmented appointments
Freiburg's dedicated VHL clinics coordinate neurosurgery, nephrology, genetics, ophthalmology, radiology, urology and other specialties. A coordinated plan can clarify which consultations are needed now and which can remain local.
International-patient estimates should follow record review
Before travel, the center should review the tumor history, current imaging and previous operations to decide whether the patient needs surveillance planning, outpatient multidisciplinary consultation, surgery or systemic therapy assessment.
Send these records before requesting a VHL estimate
A longitudinal record package produces a more realistic quote.
- VHL genetic report and family history
- Serial brain and spine MRI DICOM files
- Retinal/ophthalmology reports
- Abdominal MRI or CT for kidneys, pancreas and adrenals
- Prior pathology and operation reports
- Renal function and nephrology/urology records
- Pheochromocytoma biochemical testing if relevant
- Pancreatic NET records if present
- Audiology/temporal bone records if relevant
- Belzutifan history or current systemic-treatment question
- Requested service: surveillance, CNS surgery, renal treatment, belzutifan review, retinal treatment or multidisciplinary second opinion
