Proton and Ion Beam Therapy in Germany: Heidelberg HIT for International Patients

Proton and carbon-ion therapy are highly specialized forms of radiation treatment used for selected tumors. They are not automatically superior to conventional radiotherapy for every cancer. Heidelberg University Hospital's Ion Beam Therapy Center (HIT) evaluates specific indications where particle therapy may offer a meaningful dosimetric or biological advantage, especially when tumors are deep, resistant to conventional radiation or close to sensitive normal tissue.

Why particle therapy is selective

The physical dose distribution of protons and heavier ions can reduce radiation exposure beyond the tumor compared with conventional photon radiation in selected situations. Carbon ions also have different biological effects. Whether that advantage matters for an individual patient depends on tumor type, location, previous radiation and available alternative treatments.

HIT treats defined adult and pediatric indications

Heidelberg publishes a current list of tumors treated with proton and/or ion therapy, including selected skull-base tumors, salivary and ENT tumors, prostate cancer, brain tumors, pancreatic and liver cancers, recurrent rectal cancer, lung cancer and selected pediatric tumors. Inclusion on a published indication list does not mean every patient with that diagnosis is suitable.

Radiation planning requires detailed imaging

Particle therapy is planned using precise imaging and immobilization. Prior radiotherapy is especially important because the team needs to understand previous dose distributions and whether additional radiation can be delivered safely.

Records to prepare for a HIT review

A useful request should contain enough information for radiation oncologists to assess indication and planning feasibility.

  • Pathology and molecular diagnosis
  • Recent CT/MRI/PET imaging and reports
  • Previous radiotherapy plan and dose records
  • Operative reports
  • Systemic-treatment history
  • Current disease stage and performance status
  • A clear question about proton/carbon-ion suitability or re-irradiation

Treatment length and travel planning vary

Radiotherapy is usually delivered over multiple sessions, and the number depends on tumor type and protocol. International patients should not book a short fixed stay until the center has confirmed the treatment concept, planning appointments and expected course.
