A cochlear implant is not simply an ear operation. The pathway includes specialist hearing assessment, evaluation of whether a hearing implant is appropriate, surgery, device activation and structured rehabilitation. German university centers also distinguish cochlear implants from other implantable hearing systems, so the correct device depends on the type and severity of hearing loss.
The first step is implant candidacy assessment
Charité describes a dedicated hearing-implant pathway beginning with medical history, ENT examination, hearing testing and counseling. Previous hearing tests and treatment records should be available so the team can determine whether a cochlear implant or another hearing solution is appropriate.
Records to prepare before travelling
For an international review, collect the evidence that defines the hearing problem and previous treatment.
- Recent audiograms and speech-audiometry results
- ENT reports
- Hearing-aid history and benefit
- CT/MRI of the temporal bones or inner ear if already performed
- Previous ear surgery reports
- For children: developmental and hearing-language assessments

Surgery is only one stage
After implantation the device must be activated and programmed, and rehabilitation is important for adapting to electrical hearing. Freiburg explicitly describes rehabilitation after cochlear implantation through its Implant Center.
Plan programming and rehabilitation before returning home
International patients should ask how many initial fitting sessions are expected, when activation occurs, whether later programming can be performed locally and what rehabilitation resources are required in the home country.
Children need a coordinated long-term pathway
For pediatric patients, hearing development, speech-language rehabilitation, schooling and local audiology support are particularly important. Families should plan continuity of care before deciding to travel for implantation.
