OCT defines the macular hole and its vitreous relationship
UKE describes OCT and fundus examination for vitreofoveal traction, a vitreomacular interface disorder that can progress to a macular hole. For a specialist review, the OCT should be recent enough to show whether traction is still present and how the central retinal anatomy has changed.
Not every vitreomacular interface finding means the same operation
A macular hole must be distinguished from vitreomacular traction, epiretinal membrane and lamellar defects because their natural history and treatment decisions differ. Freiburg lists macular hole among indications considered for vitreoretinal surgery, with the decision depending on the clinical situation rather than the diagnosis label alone.
Vitrectomy is a core surgical treatment
University Hospital Cologne states that vitrectomy is used for macular holes. Its recent retinal-surgery report also describes modern sutureless vitrectomy with short-acting gas tamponade as an established approach. The exact technique, whether an internal limiting membrane peel is planned and the choice of tamponade are surgeon-specific decisions.
Gas in the eye changes travel planning
If intraocular gas is used, flying and significant altitude exposure can be unsafe until the gas has sufficiently resolved. International patients should obtain written instructions on positioning, eye drops, warning symptoms, follow-up timing and the date from which air travel is permitted.
Prepare imaging and a symptom timeline
The German center should be able to judge anatomy, duration and previous treatment before offering a plan.
- Recent macular OCT images and report
- Current visual acuity and refraction
- Approximate onset and progression of central blur or distortion
- Amsler-grid findings if documented
- Fundus photographs if available
- Previous vitreoretinal or cataract surgery
- Lens status
- Any previous macular-hole operation and tamponade used
- Current eye drops and relevant systemic medication
