Core records for almost any ophthalmology review
Include the date of each test and identify which eye it belongs to. Recent original reports are usually more useful than an undated summary.
- Current visual acuity and refraction
- Working diagnosis and symptom timeline
- Recent ophthalmology examination or slit-lamp report
- Current eye drops and systemic medication
- Previous eye laser or surgery reports
- Relevant systemic conditions such as diabetes or anticoagulant use when applicable
Cornea / keratoconus / Fuchs / transplant
- Corneal topography or tomography
- Pachymetry / corneal thickness
- Endothelial cell measurements when available
- Slit-lamp findings and corneal scarring or edema description
- Contact-lens history for keratoconus
- Previous cross-linking, DMEK, DSAEK, DALK or keratoplasty reports
Glaucoma
- Intraocular-pressure history with dates
- Visual-field tests from more than one date when available
- OCT optic nerve / RNFL / ganglion-cell analysis
- Corneal-thickness measurement
- Angle assessment or gonioscopy if already performed
- Previous SLT, MIGS, trabeculectomy, drainage implant or other glaucoma procedure reports
Retina & macula
- Macular or retinal OCT images and reports
- Fundus photographs or wide-field imaging when available
- Fluorescein angiography or OCT angiography if already performed
- Ultrasound report when media opacity prevented a clear retinal view
- History of injections, laser, vitrectomy or retinal-detachment surgery
- If gas was used previously, the operative report and tamponade details when available
Cataract & lens surgery
- Current visual acuity and refraction
- Cataract / lens examination findings
- Biometry or IOL calculation if already performed
- Corneal astigmatism measurements
- Macular OCT when available, especially if retinal disease is suspected

Send usable files, not only screenshots
If the hospital asks for imaging, provide the original exported files or full PDF reports when available. Keep test dates and eye laterality visible. Ask the hospital which secure upload channel it accepts before sending sensitive health information; avoid relying on informal messaging accounts for clinical records.
Official clinical sources
The checklist is an organizational aid, not a substitute for a hospital's own submission requirements. Individual departments may request additional tests or repeat older imaging.
