Short answer: HoLEP is a transurethral laser procedure used to remove obstructing prostate tissue in selected patients with benign prostatic hyperplasia (BPH). It is one of several treatment options and should be chosen after evaluating symptom severity, prostate size and anatomy, urinary retention, medication response, bladder function and the patient’s priorities regarding recovery and sexual side effects.

BPH symptoms alone do not determine the procedure

Lower urinary tract symptoms can have several causes. Before surgery, the urologist should confirm that prostate obstruction is a major contributor and assess whether medication, another minimally invasive procedure or surgery is most appropriate.

How HoLEP works

HoLEP uses a holmium laser to enucleate obstructing prostate tissue through the urethra without an external incision. The removed tissue can be sent for histological examination.

What to clarify before treatment

Ask about the expected catheter time, hospital stay, bleeding risk, temporary urinary symptoms, continence, retrograde ejaculation and the likelihood of needing further BPH treatment later.

A remote review is more useful with objective urinary information.

  • Symptom history and previous medication trials
  • Prostate volume from ultrasound or MRI if available
  • PSA and relevant laboratory tests
  • Urinary flow and residual urine measurements if performed
  • History of urinary retention, infection or bladder stones
